This agreement is entered into by and between Adrienne Bullock, doing business as Adrienne's Love, hereafter referred to as "Doula," and the undersigned Client, hereafter referred to as "Client."
1. Services Provided:
The Doula agrees to provide private birth doula services to the Client. These services include:
Prenatal Support (2 Visits):
Emotional and physical support during pregnancy.
Assistance with creating a birth plan and exploring birth options.
Education on pregnancy health, nutrition, and wellness.
Labor and Birth Support:
Continuous physical and emotional support during active labor and delivery.
Advocacy and support in navigating medical options.
Use of comfort measures, such as breathing techniques, position changes, and relaxation tools.
Support for both the birthing person and their chosen birth partner throughout labor and delivery.
Postpartum Support (2 Visits):
Emotional support and adjustment guidance for the new parent(s).
Breastfeeding education and assistance.
Newborn care guidance and basic household support related to newborn care.
Information on postpartum healing and wellness.
These services will be delivered in scheduled visits and support sessions, with times mutually agreed upon by the Doula and Client.
2. Payment Terms:
The total fee for the above-described birth doula services is $1200.
The Client may choose one of the following payment options:
Option A: Full Payment
Option B: Installment Plan (Three Payments)
$400 due at contract signing
$400 due halfway through pregnancy
$400 due one month before the estimated due date
All payments are non-refundable once services have been rendered or reserved. Payments can be made by cash, check, or electronic transfer as agreed upon by both parties.
3. Refunds and Cancellations:
If the Client cancels services before labor support begins, a partial refund may be considered at the Doula’s discretion, based on services already rendered (including prenatal visits and communication time). If services are canceled after labor support has been provided, no refund will be issued.
4. Duration and Scope:
This contract includes 2 prenatal visits, labor and birth support, and 2 postpartum visits. Services are considered complete after the final postpartum visit, unless otherwise mutually agreed in writing.
5. Confidentiality:
Both the Doula and the Client agree to maintain confidentiality regarding any personal, medical, or family information disclosed during the course of services. All private health information will be handled in accordance with HIPAA and standard ethical practices.
6. Liability:
The Client understands that the Doula is not a medical professional and does not provide medical care or advice. The Doula's role is strictly non-clinical and focused on emotional, physical, and informational support.
7. Agreement and Signatures:
By signing below, both parties agree to the terms outlined in this Agreement. This contract represents the entire agreement between the Doula and the Client and supersedes any previous understandings.
